Privacy Policy & Patient Consent
About this policy
Reset Physiotherapy Brunswick keeps your health information private, uses it only to care for you, and never sells it. This policy explains what we collect, how we use and protect it, and how you can access it.
Reset Physiotherapy Brunswick is operated by Chelsea Lee, physiotherapist (AHPRA registration PHY0002887966), as a sole trader ABN 81872495831.
Address: 1 Edward St, Brunswick VIC 3056 (inside Northern Myotherapy)
Email: hello@resetphysiotherapybrunswick.com
Phone: 0495 007 891
We handle your information in line with the Privacy Act 1988 (Cth) and the Australian Privacy Principles, the Health Records Act 2001 (Vic) and its Health Privacy Principles, and the Ahpra and Physiotherapy Board of Australia Code of Conduct.
What we collect and how
We collect only what we need to assess, treat and bill you safely.
Contact and identity details: name, date of birth, address, phone, email, emergency contact
Health information: your reason for attending, symptoms, medical history, medications, test results, assessment findings, treatment notes and progress
Billing details: private health fund, Medicare/CDM referral details, concession card status
Referral information: letters from your GP or other practitioners
We usually collect this directly from you: through online booking, intake forms and during your appointments. With your consent, we may also receive information from your GP, specialist or another health provider involved in your care.
You can choose to use a pseudonym or not identify yourself, but this is usually impractical for treatment and health fund claiming.
Why we collect and use it
We use your information to:
assess, diagnose and treat your condition, and keep accurate clinical records
communicate with your GP or referring practitioner about your care (with your consent)
process payments and health fund, Medicare or insurer claims
send appointment confirmations and reminders by SMS or email
meet our legal, professional and insurance obligations
We will only send you newsletters or marketing if you opt in, and you can unsubscribe at any time.
AI-assisted clinical notes
I may use an AI scribe Splose to draft notes from our conversation, so I can focus on you instead of typing. I review and approve every note, and all clinical decisions are mine. Audio is deleted after the note is made, data is stored securely and it is not used to train AI.
It's optional. You can say no or ask me to switch it off at any time, and it won't affect your care.
Who we share your information with
We share your information only when it's needed for your care, when you've agreed, or when the law requires it. This may include:
your GP, specialist or other health practitioners involved in your care
private health funds, Medicare, or insurers when you make a claim
our software provider Splose, who store and process data on our behalf under confidentiality obligations
regulators, courts or authorities where required by law, or to prevent a serious threat to someone's life, health or safety
The clinic shares a reception and waiting area with Northern Myotherapy. Northern Myotherapy is a separate business and does not have access to your physiotherapy records.
Storage and security
Your records are stored electronically in Splose, our secure, password-protected practice management system. Payments and health fund claims are processed through HICAPS. We do not keep paper records; any paper forms are scanned and securely destroyed.
We use strong passwords, two-factor authentication and up-to-date devices to protect your information. Under Victorian law, we keep adult health records for at least 7 years after your last visit, and records of patients under 18 until they turn 25. After that, records are securely destroyed.
If a data breach is likely to cause you serious harm, we will notify you and the Office of the Australian Information Commissioner (OAIC).
Access, correction and complaints
You can ask to see or get a copy of your records, or to correct anything that's wrong, by emailing hello@resetphysiotherapybrunswick.com. We'll respond within 30 days. A reasonable fee may apply for copying, but never for making the request. In rare cases the law allows us to refuse access; if so, we'll explain why in writing.
If you have a privacy concern, please contact us first and we'll try to resolve it within 30 days. If you're not satisfied, you can contact:
the Office of the Australian Information Commissioner: oaic.gov.au, 1300 363 992
the Victorian Health Complaints Commissioner: hcc.vic.gov.au, 1300 582 113
We review this policy at least yearly and post any updates on our website.
Your consent
By booking and attending an appointment, you consent to us collecting, using and storing your information as described in this policy.
Before any assessment or treatment, I'll explain what's involved, including any risks, and check you're happy to go ahead. You can ask questions, or decline any part of treatment, at any time. Dry needling is only ever used with your specific verbal consent on the day.
We'll ask before sharing reports with your GP or another practitioner, and before using an AI scribe in your session. Your answers are recorded in your file.
You can change or withdraw your consent at any time by telling me or emailing hello@resetphysiotherapybrunswick.com. If you're booking for someone under 18 or on someone else's behalf, you give this consent on their behalf.
